AC-21 and AR-11 for Change of Address

After the 140 approval I changed my employer using my EAD. But I did not inform USCIS about the employer change.

Now I want to report the change of address to the USCIS. In the AR-11 form, do I have to give the new employer name? What is the status (H1B or AOS) that I have to report to the USCIS?

Yes. Give your new employer name

What is the status (H1B or AOS) that I have to report to the USCIS?
Since you are using your EAD and not H1, report as AOS
