About birth certificate.............need help please!

I have a birth certificated without my name on it, as they named me several days later. And the certificate only has my mom's name on it, since that time in my country they only ask mother's name on the certificate.

1: Do i send in a certified translation of this BC without my name on it?

2: Do i have to send in an affidavit along with it?

You bet! Better, send one from yourself AND both parents, and anything else that can make the connection. Elementary school report cards with parents names on it? Childhood health records with parents names, etc.

I have a vaccine record card with my name and both my parents' names. I think sending in a copy of it along with my BC will be fine?
