A quick question about *BLANK SPACES* in DSP-122

If I do not have a middle name, should I leave it 1) *blank*, 2) write in "N/A", 3) or write in a "-" ?

Since N/A stands for not applicable, it makes no sense to use it in the middle name section - it is not that my name is not applicable or inappropriate, it is just that I do not own one.

I am trying to be very meticulous and leave as little confusion as possible.

The same for 7b and 7c. What should I type in? I mean, it DOES say "Answer all questions", whereas a blank space is not really an answer.

What did you guys, who have obtained their visas, write in in all these documents?
 
I think to avoid confusion I should leave a blank space, otherwise they might think I am John N/A Doe or smth.

I have a few more questions if anyone wants to help me..

4. Name of the U.S. Consular Office where you would like to process your application

Did you simply write "United States Embassy [YOUR CITY NAME]" or did you add an address or anything else?

6b. It asks for Names and ADDRESSES of schools but there is no place to put the addresses, or else they would be cut off when printing (I am printing this, right?).

What would be the solution for it?

The Signature and Date. Should I sign this electronically or manually (with black ink) after printing? I know an electronic signature is one's name and surname, whereas the "real" one is a handwritten depiction of one's name and/or surname. Does it make any difference?

Find out where in your country they process DV applications and write the full name of the post including city name.

You can add a sheet, or just write school name, city, country.

Print it and sign it with ink.
 
I thought you should write N/A too but I looked at the copy of my forms and just for your information, I left my parents' and my husbands' parents' middle-names blank because none of them have one. And they didn't have a problem with that. Not disputing what aos13 is saying nor am I sure if that helps your situation though since it's your lack of a middle name in question. N/A does make more sense and not sure why I left those blanks when I filled out my forms.
