In some cultures the use a middle name and it is different from the father's name. If you don't have a middle name just put “n/a”.

I am trying to help and this is not a legal advice.

It is safe to write your name as it is in your passport, which should be the same as in your birth certificate.

If you are referring to the DS forms to be sent back to KCC then you can call KCC for clarification. If already sent, you have time to amend them, again call them, write to them or email them with the correct information.

Hope this helps and good luck

My full name is also axx bxx cxx dxx exx
I write my first name as axx, my middle name as bxx, and my last name as cxx dxx exx. My last name therefore has 3 parts; this is how it's written in my passport, and it also matches my birth certificate.
I think your full name you write on all the forms should exactly match your birth certificate.
