A few questions for G325A and I-130

I am on F1 status (changed from B1 in 2006 within US, never leave US since then). Just got married with BF (just become US citizen) this month. Filling forms now to apply for green card. Have a few questions regarding how to fill some specific questions in those forms:

1. G325A for I-130 - at the bottom where to fill the blank "APPLICANT", should I put in my name or my husband (who sponsor my application)?

2. I-130
- Part B question No.14b "Did you gain permanent resident status through marriage to a U.S. citizen or lawful permanent resident?". If my husband has became US citizen when he married me and ticked No.13, then does he still need to answer 14b?

- Part C No.14 "He or she arrived as ?", should I fill in the status of last entry to US, or my current status? If should fill in my original B1, then the I-94 number and expireation date should be keep consistency with the B1, right?

- Part C No.17 "17. List husband/wife and all children of your relative." Should my husband fill my information there?

3. I-131 The question of " Class of Admission". Should I fill in my original B1 or current F1?

4. To which address of Chicago center all those forms (I-130, I-131, I-765, I-485 etc.) should be sent to?

Put in the last time you entered the USA, in this case, you must use the B2 visitor visa information and its respective i-94.

- Part C No.17 "17. List husband/wife and all children of your relative." Should my husband fill my information there? Yes.

3. I-131 The question of " Class of Admission". Should I fill in my original B1 or current F1? Visitor

4. To which address of Chicago center all those forms (I-130, I-131, I-765, I-485 etc.) should be sent to? Through USPS (certified/delivery confirmation) to:

This is becoming a little confusing

I am now filing for AOS to Permanent Resident as a USC petitioning for wife
We reside in PA and filing concurrently all the following forms:
I485, I130, I693, I864, G325A, I765, I131.
The confusion is because:

The last 4 digits are a redundant code that automatically tells post office sorting machines which mail carrier's bag to put the letters in. (the first 5 digits tell the machine which post office).

Putting either address into the post office zip code lookup at:
http://zip4.usps.com/zip4/welcome.jsp

says the correct address is:

PO BOX 805887
CHICAGO IL 60680-4120

Either address will work, because the post office scanner will overrite the 4107 with 4120 before putting it in the mail carrier's bag.

5/13/08 - Parent I-130/I-485/I-765/I-131 mailed to Chicago Lockbox
5/22/08 - checks cashed, receipt numbers from Missouri MSC
6/3/08 - receipt numbers not up on case status website yet
6/3/08 - out on vacation since 5/25 so not sure if notices arrived...
