I don't think there's any letter to be sent from buffalo, unless they need additional info before requesting your passport. You should take the medical form which is contained in your initial assessment package to a doctor. I got mine and went ahead to do the medicals. Then the doctor will send your papers to ottawa, and ottawa will forward them to buffalo. good luck.
