4th EAD approved

Just want share the info with forum folks!

My wife 4th EAD got approved today. It was submitted on 5/9. It took around 25 days for approval.

My EAD renewal appl sent recently.

2)I-140 approval receipt
3)passport boigraphic page
4)copy of DL
5)previuos EAD appvl mailer
6)Copy of I-94

Do we need to send the copy of 140 when you apply for dependant's EAD?

Not required.... but in case.... no extra stamps needed when you use USPS priority flat rate envelope.

BTW - I always use paper base filing....

What should we put under "DATE" (next to "Which USCIS Office")?
Receipt Date from EAD notification paper
OR
Card Valid From (date) on the card

Copy of DL - What is DL?

DL - Driving License.

I also applied 2 times on my own with paper filing and ready to file agian in July. I attach H1-B copy also as my driving license is based on H1-B validity.

Approved date of last EAD. If you check yes to column 11. Leave it blank for 'no'
