485 pending through 5 yr NIW for MDs

My pending 485 (ND 9/01 TSC) is based on the 5 year NIW rule for physicains in underserved areas and my five years are coming to an end. Does anyone know about the need for submission of documentation to INS : what/where/when etc about completion and whats the next step at this 5 year mark of completion. Or will they just ask about the current and last few years of employment (to ensure completion of commitment ) when they look at my 485 in ?? months or years ?
