485 documents question - responses appreciated

From one of the immigration sites:
Supporting documents needed for primary applicant:

Copy of passport with copy of I-94(both sides).
Copy of I-797 Notice of Action. It is an H1B/L1 visa status document.
Copy of birth certificate.
If no birth certificate or problem with it, see Affidavit of Birth
Copy of degree certificate.
Copies of recent pay stubs.
Six Immigration Photographs (2 each for I-485, I-131 and I-765).
Copy of approved Labor Certificate
Copy of approved I-140.
Original Employment letter from the employer saying that the job is still available. It should be on the company stationary.
If applicable, the applicant must also submit copies of all marriage certificates.

Copy of passport with copy of I-94 (both sides).
Copy of I-797 Notice of Action. H4 visa status document.
Copy of birth certificate.
If no birth certificate or problem with it, see Affidavit of Birth.
Copy of marriage certificate.
Original Bank Statement.
Six Immigration Photographs (2 each for I-485, I-131 and I-765).


Questions:

1. Is it required to submit a copy of my spouse’s ITIN card (as she doesn’t have a SSN) along with her 485 application?
2. Is an original employment letter must?
3. Original bank statement: Should this be a joint account for applicant and spouse? Is there any minimum amount criterion for the balance in the account?
4. I was with another company and will be with the sponsoring firm which got the 140 approved from June 1 and the 485 will be filed around june 7. I also have the WAC number for H-1 transfer for the sponsoring firm. Will the WAC letter alone be sufficient, or is the Emplyment letter a must? (not that i cant get it, just that the attorney says it is not a MUST)

It's from my own experience, therefore may not be universal.

1. ITIN not required. Never had any for my spouce.
2. Probably it's not must with application, but better to provide it. If you don't provide with your application, probably you get a RFE later anyway - asking for the same.
3. Bank statement not required. Bank balance is not a issue here. Normally it is provided just as proof of valid marriage. Therefore, if you provide, it has to be a joint account (with both name on statement). I don't think it's mandatory to provide. I never did. If you don't have joint a/c, other valid documents could be rental agreement (both names in agreement) or utility bills (again in both names). If you don't have any of them, forget it. Apply without these documents. If they have doubt (most of the time they don't) they will generate RFE or call for interview. And you will be able to straighten up your documents meanwhile.
4. Trust your attorney. I am not an expert.
 
ADIT photographs question

Will it be a good idea to write on the back of the photograph the name and passport number lightly ?
Has anyone done this ?

Should be OK, I believe.
