Hi. I've got 2 queries asking for hopital record of birth with my name and parents name mentioned and medical records with my name and parents name mentioned. I had earlier submitted only the municipality certificate of birth with parents name in it. So how do I get my birth certificate and medical records since in India we do not have any medical records of our visits to the hospitals and I do not know if we have hospital record of our birth.

you should ask your lawyer, he might have a document where your mother and father or some elderly person has to testify that you born on so and so date and notarize the form. Since I did not have official birth certificate, I had do do that too. It will not be any problem.

While i filed my 485 my attorny added afidavits to support my birth certificate . Because in India in common we wont take birth certificates on the actual date of birth . So your relative with same last name can give afidavit on behalf of your birth . that is more than enough
