485 assistance needed

My I140 has just been approved.
I need to file 485/EAD/AP immediately
Can you please advise on the list of forms/documents
needed for this process?
 
I have submitted following docs

I have submitted following docs.

1. Copy of I-140 approval
2. I-485 form
3. I-765 (EAD)
4. I-693 (Medical, this is a sealed envelope given by Doc. after the tests don\'t open it )
5. G-325A Biographic Info.
6. If you are applying for AP then you need to apply AP also.
7. Birth cert.
8. Marriage cert.
9. Last three pay stubs
10. last two years W-2
11. My H1
12. My wife\'s H4
13. Four photographs each for us
14. All pages of your and your wife\'s PP including I94
15. Plus Two Checks of $220.00, two checks of $100.00, two checks of $25.00 ( $220.00 for I-485 + $100.00 for I-765 and $25 for Finger Printing) + 2 checks of 95.00 for AP if you are applying. (Assuming you are married otherwise one check each)

I didnot submit "Affidavit of Support (I-864)". But I think you need it if you read through other posts. Lucky You.

What is the cost of medical per person and how long does this take?

Regarding the medical, the doctor, that I went to, put the expenses on my insurance and all I had to do was pay the co-payment amount.
