Hi Guys, I have received the 2nd RFE about the Medical Exam. It says that "The supplement to the form I-693 (Medical Exam) had not been signed by the civil surgeon who conducted your exam. The civil surgeon must sign and date the supplement to form I-693 where marked in RED. Photocopied signatures are not acceptable."

I don\'t remember the doctor/facility name in Baltimore, MD that I have done the Medical Exam. Does anyone have the civil surgeon listing who has been designated by the INS in Baltimore, MD. I will really appreciate it.
