2 yr HRR calculation

I am a physician who completed my training on a J1 in the US - thus have a 2 yr HRR. I am currently working in a third country but spend a significant percentage of time at my home country.

My fiance is in the US and is planning to apply for a K visa for me.

How is the 2 yr HRR calculated - do they actually calculate each trip made outside of the country and add it to the time period for the HRR?

Any help would be appreciated.

My J1 program visa officer told me that they really exclude the days you were in the US (whatever reasons, visiting fiance, attending confference....) from 2 year HRR. But, I did not ask the officer whether they "really" exclude the days you are in the third country from 2 year HRR.
