Q1:
What rule control where to get stamped once 485 is approved? I'm in South Carolina waiting for approval of 485. I know there is one USCIS Greer office around upstate of SC. Will that be the office for stamping PP?
Q2:
Is medical mandatory or you can skip it in the hope that no RFE issued for it ?

when you get approved, there is loacal office on your notice, also as i understand, you can go anywhere to stemp it

Q2:
i did not understand the question clearly, medical exam is only good for one year, because long backlog, USCIS has determined the medical exam will be as good as you get approved, all 2001 filer' medical exams are all over 2 or almost 3 years already, no problem at all. most RFEs focus on employment if you are EB.
