180 day rule for Physicians?

My brother is an MD. He has completed his 3-year period on H1 in an underserved area. He currently holds a self-petitioned appoved NIW. He filed for his AOS/AP/EAD couple months back. His EAD and AP have been approved recently. He will complete his 180 days some time in Feb 2002. My questions are:

- Does he qualify to accept a job offer after 180 days in another underserved area (in a different state/Service Center) using his EAD without having to give up his pending I485?

- Can he use his remaining 2 years on H1 (instead of EAD) to join the new employer without having to give up his pending I-485?

- If he has to give up his I-485, what does he really need to do to be able to use his NIW to apply for a new AOS?

He will need to get a new NIW ( I-140) approved after getting a new "public interest" letter from health dept( of the new location) for the new underserved location. he will however be able to count those 3 years to the total requirement of 5 years. if he changes jobs and does not get a new NIW approval he will be in trouble. the NIW for physicians is pretty location specific and does not give much room to change jobs. the rules for physician NIW are clear on that.

Can a qualified doctor(J1/waiver/ H1 /3 year underserved area finished) start his own practice based on the EAD from spouse.
If yes what are the formalities to be done for starting work on an EAD. Which agency to be informed? etc.
