10.99 and W-2 on N-400

Dears:

I'm filling out N-400 form, and need to mention in my employment history all the places where I work. Technically, I need to disclose all the places where I worked and then reported to IRS.

I have W-2s and 10.99 forms to disclose. I understand that when you get W-2 form you are employed with company, but when you get 10.99 you are considered self-employed.

The question is: How do I exactly describe 10.99 form?

Say I got 10.99 every year for 5 years in a row from ABC Inc. I worked as a writer. Should put it like: Self-employed as Writer from 01/01/2004 to 01/01/2009. Name of company ABC Inc. or simply Writer from 01/01/2004 to 01/01/2009 at ABC inc.

"Self-employed as Writer from 01/01/2004 to 01/01/2009" seems about right.

The N-400 Instructions for Part 6B say, "If you worked for yourself, write ''self employed.''"

I think you should be good to go with just that.
