In a packet I got for the interview, there was no medical exam form, only the instructions. Is the form downloadable from somewhere (in case they forgot to put it in the packet), or is it expected from doctors to have it? Thanks!

Not sure which form too use. I-864 seems to be only for relatives. Also, is the company letter stating salary enough, or you need to send the bank info too. The instructions say "any or all" of the two requirements. Thanks!

I submitted DV 2005 winning entry application to KCC in second half of May. Does anyone know their processing times? Phone number? E-mail? Do they respond to inquires?
