No Title the 9003 form is necessary. the man at the counter specifically took the document. it is a yellow piece of paper, pretty standard to fill out. in fact, in my experience, they kept the 9003 form, but did not look at the tax returns at all.

No Title I had both part I and II. actually, my part I form was filled in incorrectly by the lawyers (it had a "DRAFT" stamp on it), so i had to refill it out at the consulate. the part II form is the one they make you sign if you are approved.
