the 9003 form is necessary. the man at the counter specifically took the document. it is a yellow piece of paper, pretty standard to fill out.
in fact, in my experience, they kept the 9003 form, but did not look at the tax returns at all.

I had both part I and II. actually, my part I form was filled in incorrectly by the lawyers (it had a "DRAFT" stamp on it), so i had to refill it out at the consulate. the part II form is the one they make you sign if you are approved.
