Thanks mothkuri. My lawyer do not have any idea!!! I will carry both the passport. My all documents in India contain the old name. My all document in usa contain the new spelling including the gc application. So I corrected my passport to the new spelling.

Bappa: My lawyer is preparing the forms today. I will receive on tomorrow. By saturday I will post the contents and instructions in details in this forum
