Hi, I-140 and I-485 were not filed concurrently (I filed for I-485 just before INS allowed concurrent filing). I-140 was approved 2 years ago. I don't know what you mean be labor substitution. I have used AC-21 to switch jobs recently though.

Why are you making this difficult for yourself? Just fill out the following form: http://www.bcis.gov/graphics/formsfee/forms/i-131.htm follow the instructions there. Give them two ADIT style photos, a copy of your I-485 receipt notice, and the filing fee, and you should be good to go.
