Hi Jackolantern, No, she is a US citizen by birth, and she's never taken any legal action to change her name. A legal marriage then, does not count as "taking a legal action" so I am going to use the name that appears on her birth certificate.

Hello again, Ok, I called the US CIS service toll-free number, and they said I should put the "name given at birth" or "name taken with legal action". My wife uses the same name everywhere, driver license, bank accounts, mortgage, loans etc. Wouldn't that be her "legal name"?
