1) I got my name change during my interview, last name is still the same, first name is difference, and i also got a form called petition for name change which listed my old and new name. does anyone change their name during an interview have that form? Do i need to bring that form with me...

i got a warning ticket (pay fine and attend court are not require) but said no in my application. Am i in trouble? should i mention it during the interview even if the officer might not ask the question?
