Well, there could be number of reasons for I-485 denial....It could have been triggered by one of the three buckets (1) Labor denial (2) I-140 denial (3) I-485 denial

- Fraudulent Labor Filing
- Lawyer Fraud
- Employer fraud - Black-listed employer
- Reuse of Labor for...
Firstly, your employer cannot withdraw your I-485 application....
he can only request to USCIS for revoking/withdrawing the I-140 request...and it may be possible that USCIS may revoke your I-485 consequently....based on underlying I-140 withdrawal...

You can try visiting your nearest local CBP office for a duplicate copy possibly? (CBP - Customs & Border Protection Agency)
I think - it's a non-issue.....Lot of people forget to return their I-94 when leaving the country...You may have trouble getting Driving License and other federal stuff...

I just used sworn affidavits from Mom/Dad/Uncle as proof of my birth and sent them to USCIS and got approved....

Usually the person responsible gets the RFE on his/her I-485....If you both got RFE...then it might 2 different RFE's on each of your case....
Traditionally RFE's on I-485 fall into 3 buckets....
1) EVL - Employement Verification Letter
2) Proof of Legal Status - I-94, Birth Certificate...
I agree with Jackolantern....In the new revised structure - renewals are free.....that's how USCIS justified their fee hikes....saying....it's really not that expensive....
But, I agree, this information is still shrouded in the bills...etc....and not clearly spelled out on the I-765 /I-131...
In most cases - 90% of the time - RFE means the I-485 has been pre-adjudicated and in final stages...and they are just trying to close the file by clarifying one scenario and should close in 2 weeks or less - if RFE is responded....
If your PD is not current - it really does not mean...
RFE could be anything....these are the most common,
1) EVL - Employment Verification letter
2) I-94 - status inquiry on the person to prove lawful status since last entry into US before filing I-485

The only reason DOS moves dates back is - when they have no clue....and they just want to close the doors for all....
That's what happened for EB2-IND .....they had monthly allotments for EB2-IND = (10,000 / 3) - 3333 / 12 months = 300 each month for EB2-IND....

Traditionally, congressman or senator's office .....will contact their liaison and create an inquiry ticket on your behalf with TSC/NSC.....Let's say you went with congressman.....and they created a ticket.....if you contact senator - they will...

If he is a screwy guy...he may issue an RFE....
Bottomline, there is no reason for them to issue the RFE for EVL (Employment Verification letter).....The only reason they issue it - there is too much itch up the adjudicator's AS*....
But, yep...
"Document mailed to applicant" means that one of USCIS notices....did not get delivered to your address and went back to USCIS....
It's usually - Address Change Confirmation Notice, Fingerprint Notice etc......
Now, USCIS - what it will do - it will check AR11 database for updated address...
I believe, if you use EAD once - you abandon the H1B status....and cannot come back to H1B....
if really want to come back...may need to apply under the H1B lottery system....
You either need to stick to H1B all along....or just abandon H1B and stay on EAD all along....
