Higher RFE ratio for premium cases may have nothing to do with USCIS having more scrutiny. It could simply be that people that are upgrading to premium have problematic cases by default, and therefore you see more RFEs coming out of premium. But these RFEs would come out anyway through normal...
I think the 15 days, are 15 business days. And yep, you are supposed to get your money back if you dont hear from them in that timeframe, and, they are gonna keep your case in premium processing.
Most of the cases converted to premium around these forums have been approved without RFE. For that matter, I haven't seen any differences in the ratio of RFE/approvals for upgraded cases. Besides, I really doubt NSC uses any different criteria for premium cases than regular cases, they are just...

Premium processing was expanded on 09/25/06 to accept EB1 and EB2 petitions. The fee for premium processing is $1000, you are promised an answer (approval or RFE) in 15 days or you get your money back. At current processing rates you should have to wait around 2 months to see your case approved.

Did your I-485 get appoved too? I always wonder why the USCIS doesn't approved concurrently filed cases if all the processing is done with the I-485 (fingerprinting, EADs, APs..). It would save time for everybody else, instead of now picking up your case and putting it into another queue.

I'm not sure why you say there is progress in the EB2 arena. I don't see much movement anywhere. Seems like NSC has been stuck in April for a long time. TSC zooming by this is just ridiculous.

I am sure that an RFE on the I-140 would not change any extension eligibility. but yeah, He should be pretty close to hear something from the USCIS so maybe its not worth it to pluck down the $1000.
Why shouldn't you upgrade? If you get an RFE you'll get it with or without premium processing, and its better to get it before than after so you have time to react. So go for it..
Yeah, in my case the I-140, the I-485 and the EAD got the same LUDs on consecutive days.. This could very well be someone scanning a barcode attached on the cases or something like that. At least they are somewhere and not lost in a corner getting dust.
