Could somebody please tell me 1) whether I need to print the DS-156 Form on a single piece of paper (front and back) for the visa revalidation? 2) what is the issurance date for the revalidated visa? Is it the Washington receive date, or is it the date when the visa is actually printed...- NYCH1B
