You may need H1 Since you do not have any EAD, you may have to take another H1. If you have EAD, you can make use of it.

EAD has A# Your has EAD card shows the A#. Call VSC and give your A# and ask for EAC #.

No Title I think you will have to prove that she had legal status all the time during her stay in USA. Probably, you should have all I-94s. If at any time, she is not having legal status, I think you should pay $1000/- penalty. I have read some postings on this board similar to this one...
