The employment letter has to be notarized,it ia an unwritten rule. It is the company who has to get it notarized.Genrally the person who issues the letter has to sign the employment letter in front of the notary public..
In case of the I134 it is you who has to get it notarized.

You really don't have to give any reasons for extreme hardship. It is at the consulates discretion.

From what I have heard and read.They take a look at your passport to check if you have a valid H1. If it has expired they ask for the approval notice only to check if you are not out of status while in the USA. If you have a receipt notice while you were in status you should be ok.
Apply for consular processing during the i140 stage...you will be good to go..no need of doing ac140....this is only for people who apply for AOS and then convert to CP BY FILING 824(and then send the papers directly to the consulate thru the attorney claiming hardships). Hence the term Attorney...
