Docs required for H4 visa stamping

Hi all My wife is in India and she has to go for H4 visa stamping. What documents are needed? The reason I am asking is, I got a new H1 approval and I did not get my stamping done yet. And she has the H4 validity till the end of the year. Please respond. Thanks Satish
