NIH research fellow on H1 to clinical fellow

Dear Guys, I'm NIH research fellow and I'm supposed to get on H1b shortly. Is it possible to swich to clinical fellow within NIH while keeping my H1b? Or should I give up H1b and get clinical J1 and suffer from 2 year HRR in the future? sakatake
