As an Asylee, do I submit the I 693 (Medical Examination) together with I 485? Or do I need to wait till they ask for it?

Please help....I hold a RTD and want to go to Europe next year for tourism. If I enter through Germany, do I automatically have acces to other Schengen countries or do I need to ask for a visa for one of those countries? I have no idea how it works. Your ideas, suggestions and past experiences...
