2). W2 Tax Form (Scanned Copy) - http://www.irs.gov/uac/Form-W-2,-Wage-and-Tax-Statement
3) . Form 1040

Hi Kasun, You can find more about Certification of UK qualifications here (http://www.britishcouncil.lk/study-uk/certification) I paid LKR 4000 (2000 x 2) for my degree certificate and transcript.

It's not necessary to provide the verification from British Council. But In my case I did that because qualifying on work experience is very difficult .

I'm DV winner 2014 from Sri Lanka, my interview is on Sept, has anyone went to the interview and got through by only showing the form i134.
