I am sponsoring my married sister who changed her name after marriage and she does not have a marriage certificate. Is there any affidavit I can provide to prove her name change ? where do I find the sample format for that ?

I would like to sponsor my sister for family based green card. She does not have a birth certificate. Is there any way her passport or her school certificates may be used to derive a birth certificate? She is an indian citizen. Any help will be appreciated.
