Take both copies with you. There is nothing to do with AP when entering India.

AP has nothing to do with status. it is travel document only. if the person is on H4 and not working/intend to work, EAD does not matter. He/she can travel on AP and still maintain an non-immigrant status. I am assuming that a I485 is pending for this person.
Should not be a problem. All the INS agents are familiar with the I551 stamp. It is same as the actual GC. I confirmed with the Canadian consulate and they said it was no problem.

More than likely your 485 will be approved before feb 2003 and you wont have to apply for any extension. Since your EAD is valid till May 2003, I see no reason to file for H1 extenstion.
