visitors visa cancelled CWOP

My nephew who is studying in St Kitts(Carribbean Island) had a 10 year visitors visa to USA since 2 years. He always went to India via USA. This time when he was enroute to India via USA he entered Puerto Rico. He was stopped for a long time. First they questioned him as to why he always always pasees through USA. He explained to them that he does so becasue tickest are cheaper besides the fact that he has friends and relatives in USA. He missed his connecting flight to Chicago. After few hours they came back and said that that there was a mistake by the US authorities at thh time of visa was issued two years back and they are cancelling his visa with CWOP(Cancelled without Prejudice). Apparently there have updated their computer systems and therefore could not catch this mistake in his earlier visits. However they let him in this time for 6 months. He will now have to get a new visa before he reenters.

Do you think there is more to it than meets the eye here? What are his chances of getting the visa next time.
