Can I change to H4(My wife's H1) and stay in US after 6 years?

No. All time in H counts toward th 6 year limit.

Being said that, if your wife was on H4 from the begining of your H1, here H1 should be finishing 6 yrs. right about your time.

If that is the case, then how would a spouse get H4 extension for 7th year based on 7th year H1 extension ?

7th year extension is different story.

If primary's H1 is finishing 6 yrs. and the secondary was on H4 for 3 and H1 for 3, then secondary would not get another H1 extension.

In 7th year, if primary's H1 is extended, secondary is eligible for H4 extension , but not H1 extension.
