Find following the format of the fax for FP request.

Now, PPP days have already passed and I have not yet received the finger printing notice.
At this time I would like to request you to issue me the Finger Printing Notice.
My case details are as follows:

An immediate action on this case would be highly appreciated.

2. Receipt # : SRC-02-xxx-xxxxx
Applicant : your spouse’s name
Receipt Date :xx/xx/02
We would really appreciate if you could send us the notifications soon.
