Medical Examination Forms

Dear Friends,

My interview is on October 3, but I haven't got the second letter. Therefore I don't have the medical forms and I don't even know the authorised medical centres in my city (London). I called US embassy but they gave me an email address to contact. The auto reply said that the averaging respond time is 4 days and I am out of the time. Could anybody please give me any information about the forms and medical centers. Thanks
 
you don't need the forms. The doctors will have it.

Actually I have been waiting for the second package so far.
In US embassy website it is mentioned there are two authorised center but they didn't mentioned their name and address.
 
Authorised Medical Practioners

I think they will reply you soon. I sent email to USA embassy in my country. They replied within two days and provided the names of authorised Medical Practitioners.
