married with children cpers pls. help!

My amateur attorney has no clue on filling Affidavit of support. When I have 3 dependents: wife and 2 children can I use one form to fill or need 3 individual forms one for each member cos in item 3 it says "...on behalf of person" and not persons. This form is turning out to be a pain-in-the-butt.

Affidavit of support - 134 form

If you read the instructions page of the 134 Affidavit of support form, the first line says, “A separate affidavit must be submitted for each person”. I guess it is quiet unambiguous.

if i am putting my wife\'s name in item 3, again they ask spouse\'s name do i put my name or my wife\'s here? this "spouse" seems repetative here
