After get GC, can GC holder leave US for more than 6 months continually? I thought that would only have impact on the natualization later but heard that this may violate the GC status. The border officer may suspend the GC and not allow to enter when return US after 6 menths leave. Is that true?

If you stay out of US for more than 6 months , you will have trouble when you come back, unless you apply for a re-entry permit before departure. You dont have to wait for its approval though.

For stays upto 1 year, only the green card is required to re-enter US. If the stay is longer than 6 months you may be questioned to verify that you have not abandoned US as your residence. YOu will not be denied entry if you have not abandoned your residence and are not excludable due to some other reasons (diseases, crime, etc.). Stays more than 1 year require approved re-entry permit or a returning resident's visa from a consulate.

One has to maintain a permanent residence in the US throughout, even if the trip overseas is shorter than 6 months.

What happens if ---- application for reentry permit was made AND approved --- however, actual period of stay is more than 2 years, say 4 years. (due to Medical as well as family reasons !!)

Is it advisable to seek "returning residents VISA" from the consulate?

You will most likely lose your green card if you stay for 4 years outside US. It is almost impossible to establish permanent strong ties to US in such cases.
Returning residents visas are granted only under very compelling circumstances. Even then you will have to satisfy the Immigration officer that you have close ties to US.
