Leaving the US for a month.

I am a Permanent Resident. I am going to Europe for a month.

Is there anything I need to do when I leave the United States? Fill out paperwork or something?

What do I need to do when I am coming back to the US? Fill out I-94 or something else.

I paid a fine of $100 for jumping a turnstile in the NYC Subway. Was not even a Misdemeanor. Should I be worried about being held up at the POE when I come back.

You can go and come in peace. No I94 needed anymore. Just a customs form. Your misdemeanor shouldn't pose a problem.
 
If you were ticketed than no problem, if you were arrested you might have to go to secondary to determine admissibility, which will be ok even if you were arrested, just a bit of a hassle nothing more.
