Is Hepatitis vaccine necessary?

Does anybody know if Hepatitis B vaccine is required? Which column should be ticked on this line if its not required?
Do most doctors offices type the form I-693 or is it ok if it is hand written?

Hand written is ok

Hand written is ok.
Hepatitis-B is not compulsory i guess.
But if you are a female, Rubella is a must.

The Hepatitis B vaccine takes 6 months. There are 3 shots altogether. So judging from the fact that it takes 6 months to complete it, it's probably not required - but check with your lawyer or doctor.
