Your H-4 should be tied to your spouse's I-797.
You do not need to get a fresh visa when you apply for an H-4 extension. You just will not be able to travel.

also, We are thinking of traveling back home in Asia..... can we do that? if not, what do we have to do in order to do so?

In that case, your H-4 is tied to your parent's H-1B.
