1. I have lived at several addresses in the past few years. But this is the first time I am filling form AR-11. Do I need to fill out a form for each address where I have lived?

2.I have never filed AR11 before as I did not know about it - does that have any negative impact on my green card processing or me being in USA.
 
Its important to fill AR-11 but it came into effect I believe sometime after Sept 2001. I always keep my lawyer informed about any address change and I have been asked to fill AR11 atleast for the two moves over the past two years.

But you are right. I never thought about asking for all the previous addresses I have lived at.

I lived in at about 6 addresses in US from the day i landed here. I filled AR11 for 4 of them which i changed after the AR11 form came into affect. I think you need not to give the old addresses now.
 
Please take AR11 seriously!!!!!!!

If you haven't been doing that then I would suggest talk to your lawyer and get everything straightened before the processing begins. You do not want delays if they at all start processing because of stupid address issue!!!. BCIS loves to generate RFEs. You are just giving them a reason.
Just imagine dates become current for a brief time due to miracle. Your file is being processed. BCIS generated RFE. 2-3 months are wasted and then suddenly dates retrogess. You will be devastated if that happens, especially if GC was in your reach.
