At point 25 is asking:

"List dates of all previous visits to or residence in US.
From/To (mm-yyyy) Location Type of Visa A number "

As a Canadians we entered countless number of times with out visa. Any suggestions how we can fill?

Just fill out "numerous visits as Canadian visitor".
