Do I need a name change affidavit for OCI..or it is fine..??

Hi All,

In my Indian passport, I had my name as "xxx yyy zzz" as Given Name and " " in Surname. Now in my US passport I have "xxx yyy" in Given Name and "zzz" as Surname. I am now getting ready for the Surrender Certificate, Renunciation and OCI.

Do I mention that I have changed my name..or its still the same..Do I need to go for a affidavit for name change..Confused.

You should have a name change certificate. I got one from during the naturalisation ceremony. If not, it might not be a big issue because your name is essentially the same, no?
