HELP: Fax inquiry question

Hi Folks,
I'm preparing fax inquiry sheet and need some help in filling the following infomation.

INQUIRY INFORMATION
Date of Filing ..... (ND? RD? or some other date)
Classification..... (??)

I485 INQUIRY Please Provide the following
Principal Alien's Petition Type..... (is it I485?)
Principal priority date..... (this column is empty in my I797C)
Principal Visa category..... (is it H1B??)
================

I'm beneficiary on my spouse case. Any help would be much appreciated.

Folks! PLEASE HELP.

"I-485 Employment based" for your wife
"I-485 Employment based (Derivative Adjustment)" for you


I485 INQUIRY Please Provide the following

If your wife is the principal applicant, then her Petition type. Yes it is I-485
Principal priority date..... (this column is empty in my I797C)
I just mentioned the month and year. You should have some idea about this.
Principal Visa category..... (is it H1B??)

I think RD is OK

Classification..... (??)
check page 40 for list

Then look on to I140 approval notice.
For example in my I140 - Skilled worker or professional Sec.203(b)(3)(A)(i) or (ii) which means E3 classification

Principal priority date.....
It should be on you wife I140 approval notice
