Confused please help

I read the naturalisation book and I am a little confused about the documents I need to send.I am applying ont he basis of employment and 5 years(minus 90 days).I have never had a trip outside for more than 6 months so
that means I send
copy of GC
photos and the cashiers check
right??
Also I got the approval on october 13 1999 so I can apply after July 15 2004 right.
I know this is asking too much but I just wanted another confirmation.

Application, photos, green card copy and check should be sufficient. I have sent those only
