Choice of Agent form

Hi,
I am the petitioner. I received the DS-3032 with my spouse(beneficiary) name on it.

My question is

Since my spouse lives outside US and I received the "Choice of agent form" from NVC

1.Who (Petitioner or beneficiary) has to fill and sign this form?
2. Where should this form be sent? IDoes it have to be sent to NVC in U.S or a consulate in non-US country.

Please advice.

The beneficiary signs it.

Did you receive it in the email or postal mail? Either way, it says where to send it to in the instructions (the NVC address).
