Change of job using AC-21

I have a l-485 pending through Labor cert and it has been 4 yrs since the RD of I-485. I was trying to wait for I-485 approval to move on to a fellowship from an underserved area. Can't do the waiting anymore! What are the steps I have to do to move to a similar kind of job in a different/same state? I have been working as an Internist if it helps!
