Hi,

I am a green card holder and want to go for a vacation in Caribbean. What documents should I carry with myself and what documents are required?

So I should bring my passport of my country which is (Pakistan) and my green card. And my wife is a US citizen so she should bring her US passport.
 
Yes, that is what is needed by you both to re-enter the US. Also make sure you have the required documents to enter the country(ies) you are visiting.

One or both of your may need a visa for the country(ies) you plan to visit. Not all Caribbean countries allow visa-free travel for green card holders and US citizens.
